L DIVISIONAL T-SHIRT ORDER FORM 

PRE-ORDER DEADLINE:  Wednesday, July 21

ITEMS ARE NOT RESERVED UNTIL YOU MAKE PAYMENT TO YOUR TEAM REP.  ALL MONEY IS DUE TO YOUR TEAM REP PRIOR TO WEDNESDAY, JULY 21.

  • Each T-Shirt requires a separate entry. If you are ordering more than one shirt, please submit a NEW form for each item.
  • You will pay your team rep for your shirt. The shirts will be delivered to your team rep prior to divisionals for distribution.
  • We strongly suggest that you pre-order your shirts.  Shirts may be available on the day of divisionals but sizing and quantities will be limited.
